Tiffany Riser

Tiffany Riser is a second-year doctoral student at the Johns Hopkins School of Nursing. She was awarded the Mathy Mezey Scholarship for her engagement in geriatric nursing. As a Nurse Practitioner and Home-Based Primary Care (HBPC) Program Director, she has provided primary care, palliative care, and case management for homebound and geriatric patient populations. This population is often fragile, isolated, and vulnerable with medically complex care needs and barriers to accessing care in traditional outpatient clinics.

As a current doctoral student, she is in the process of expanding her research knowledge and acquiring skills as a nurse scientist. Ms. Riser is board certified as an Adult Nurse Practitioner by the American Association of Nurse Practitioners (AANP). She completed her Bachelor of Science in Nursing degree at the University of Pittsburgh and her Master of Science in Nursing and Master of Public Health degrees at the Johns Hopkins University. As an undergraduate student, she completed a nursing research assistantship, a nursing study abroad program in organ transplantation, and an independent study in oncology nursing.

As a graduate student, Ms. Riser participated in professional organizations, student fellowships, clinical preceptorships, and research assistantships including the Anna Baetjer Society for Public Health Practice. Ms. Riser was selected as a 2017-18 fellow in the Health and Aging Policy Fellowship (HAPF) Program, sponsored by the John A. Hartford Foundation, West Health, and The Atlantic Philanthropies. Ms. Riser received the DAISY Foundation Award for Extraordinary Nurses in 2017 while working at the U.S. Department of Veterans Affairs. She has served on the Family Caregiver Alliance (FCA) Board of Directors since 2018 and was recently recognized as a 2022 Academy Health Diversity Scholar.

Ms. Riser’s interests include health services and outcomes research, implementation of home care models, social determinants of health, and health equity among aging populations. She is also interested in family caregiving policies and programs. Ms. Riser has served older adults and veterans in transition – transitions to higher levels of care, transitions to end-of-life care, transitions from the hospital to home, and transitions through medical and personal hardships. Making home visits to homebound seniors, reveals how aging adults in our communities are facing barriers to accessing care and to needed resources. A variety of social determinants have significant influence on the health outcomes of older adults and those that care for them. Ms. Riser is committed to the goal of translating her clinical experiences into meaningful research that improves health outcomes and service delivery for older adults.
